As more complex medical treatment options have emerged the case management concept in healthcare has come into existence as a way to eliminate gaps in the care provided as well as to make sure treatments were not needlessly duplicated, and control the cost of quality patient care.

The RN Case Manager will communicate with the physician and other care givers about changes in the patient’s clinical condition including results of diagnostic tests and symptomatology. The Registered Nurse Case Manager is able to respond accurately and quickly to changes in patient’s condition or response to treatment as well as be able to perform general nursing duties.

RN Case Manager has several major duties some of which may be:

Identifying the clients insurance coverage or source of payment for services.

Finding and addressing client risk factors

Documenting case management plans of the patient as well as ongoing activities

Identifying patient needs, current services and what resources may be available to them and then connecting them to these services and/or resources to meet their established goals.

Serve as the patient’s advocate and communicate their care preferences. Verify that interventions meet the needs and goals of the treatment plan.

Screening and assessment of patient.

Developing a case management care plan based on the needs of each patient.

Providing education to the patient and their family/caregiver about the case management process and trying to help them understand the process.

Providing discharge information and education to the patient and families, including medical information, nutrition and care of surgical site.

Strong computer skills and excellent communication skills, verbal and written, is essential.

Strong leadership and team building skills.

Career Requirements

Associates or Bachelors Degree in Nursing

At least one year of related experience

RN license is required. Certified Case Manager certification may be preferred.

Key skills needed would include organizational skills, critical thinking, decision making, interpersonal communication, benefits management, experience with database, medical, word processing and email software as well as knowledge of medical equipment.
