Providing care for patients in their own homes enables permanently disabled and suffering and/or ill patients to remain at home for treatment instead of dealing with lengthy, expensive hospital visits. Home Health LPN/LVN’s are an important part of this service whether they are performing medical duties or comforting a family member.

Home Health Licensed Practice Nurse/Licensed Vocational Nurses need skills beyond medical training in addition to the ability to care for the patient. Many LPN/LVN’s work independently and must be capable of making treatment decisions and performing their duties without a lot of supervision. Supervisory skills may be necessary if the LPN/LVN will be overseeing home health care aides or others providing care.

Responsibilities and Duties of the Home Health Licensed Practice Nurse/Licensed Vocational Nurse

• Provide services and/or treatments requiring specialized and substantial nursing skills.
• Initiate the care plan and update and revise this plan as required.
• Provide counseling for the patient and family in order to meet nursing and related needs.
• Prepare accurate clinical and progress notes.
• Function as an on call nurse.
• Refer to other professionals as needed.
• Evaluate and document the outcomes of care.
• Honor and protect the patient and coworker privacy and confidentiality
• Plan for the discharge from services
• Supervise other medical personnel and professionals also providing services to the patient.

Home Health Licensed Practice Nurse/Licensed Vocational Nurses will perform basic medical duties such as monitoring health statistics and giving medication at appropriate times and dosages as well as providing treatment and palliative care. Effective communication skills are necessary to explain treatments and medications to patients and family, as well as collaborating with doctors and other caregivers.

Home Health LPN/LVN will treat a large variety of patients. Age may range from newborns and young mothers recovering from childbirth to the elderly patient coping with chronic pain and illness. Patients may be suffering from serious diseases such as cancer, or be a victim of a serious injury or accident. Some may be recovering while others may be dying. Home Health LPN/LVN must be able to cope with each individual situation with compassion.
